I'm not sure why but there are 3 pictures that you posted I cannot see. 
Firstly, I don't want to seem like the biggest wet blanket on the planet but we only have normal humans here on this board. That means no people with purple eyes unless they choose to wear contact lenses. Also try to correct any incorrect words in your app. You have quite a few and if you don't know how, please, let me know. I'll happily do it for you. 
Your character is a bit horrific for this setting. Insane is a good quality to have but he seems a little fake. If you could flesh him out and explain why he is the way he is would help support his reason in being unstable. Also, schools in this world are not exactly open as they are in real life. They are scattered and so being one would mean there would be limited lessons. Him being a medic and going for his license in such a setting would be possible but he'd need to have to know the right people to do so, like military personnel or such.
I do like how you convey his madness though, with actually talking and interacting with his alternative personalities. If you want to know the setting this forum takes after, please feel free to type in 'The Last of Us' in google search so you get a better idea of what we expect here.
